Use this form to report operational issues, technical problems, security concerns, or personnel complaints. All submissions are reviewed by regional command.
⚠ MANDATORY NOTICE: All reports require valid identification and are logged in Authority systems. False reports may result in fines, detention, or travel restrictions. Reports deemed frivolous will be penalized.
Reporting Categories
Select the appropriate category for your report:
1. Personnel Complaint (Form AR-801)
Report misconduct, unprofessional behavior, or mistreatment by Authority personnel.
- Processing Time: 60-90 days
- Fee: None (initial complaint)
- Outcome Rate: 1.2% result in corrective action
- Warning: Frivolous complaints may result in $250 fine
2. Technical/Operational Issue (Form AR-802)
Report technical problems with systems, facilities, or procedures.
- Processing Time: 30-45 days
- Fee: None
- Examples: Equipment failures, website errors, facility maintenance needs
3. Security Concern (Form AR-803)
Report suspicious activity, potential threats, or security vulnerabilities.
- Processing Time: Immediate (priority review)
- Fee: None
- Confidentiality: Reporter identity protected
- Warning: False security reports carry severe penalties (up to 5 years detention)
4. Document/Database Error (Form AR-804)
Report errors in your official records or documents.
- Processing Time: 45-60 days
- Fee: None (error correction)
- Note: Corrections require proof of error
5. Policy Clarification Request (Form AR-805)
Request clarification on Authority policies or procedures.
- Processing Time: 30-60 days
- Fee: None
- Note: Responses are not legally binding
After Submission

Confirmation
- You will receive a report reference number
- Confirmation sent to provided email (if applicable)
- Reference number required for any follow-up inquiries
Review Process
- Report reviewed by appropriate Authority department

- Authority may conduct investigation based on report

- No guarantee of specific outcome
Resolution
- Written response sent to your registered address (if applicable)

- Decisions are final and not subject to appeal
Status Inquiry
To check the status of a previously submitted report:
- Contact regional command with your report reference number
- Inquiries accepted 30+ days after submission
- Status updates provided at Authority discretion
- Repeated inquiries may delay processing
Alternative Reporting Methods
In-Person Reporting
Visit any Authority regional office to file a report in person:
- Bring valid identification
- Forms available at all offices
- Staff assistance available during business hours (08:00-16:00)
Mail Reporting
Send completed paper forms to your regional command center:
- Forms available for download (see Travelers page)
- Include copy of identification
- Allow additional 2-3 weeks for mail processing
Anonymous Reporting
Security concerns may be reported anonymously:
- Anonymous reports receive lower priority
- No follow-up possible for anonymous reports
- False anonymous tips are still prosecutable if source is identified
Penalties for False Reports
WARNING: False or frivolous reports carry severe consequences:
- False Personnel Complaint: $250-1,000 fine, potential travel restrictions
- False Security Report: Up to 5 years detention, permanent travel ban
- Frivolous Technical Report: $100 fine, report privileges suspended
- Malicious False Report: Criminal charges, detention, zone expulsion
